form to attach documents such as cab cards or
spreadsheets reflecting the required truck
info(as shown on the equipment section of this
form, then change the QUANTITY at the
payment center to reflect the number of units. If
you have several units to permit, call our
office to obtain discounted pricing tiers.

Note: Your
credit card transaction will be processed by
Authorities Direct. Please EXPECT this on your next
monthly statement. The Permit cost includes a
minimum $50 NON-REFUNDABLE handling fee for all
permits regardless of states decision to grant.
Effective April 1, 2006 New York requires
all new applicants to register prior to be issued a
permit (temporary or permanent), this is included in
your permit service.
